Our website does not currently provide native submission forms for contact messages, intended parent applications, surrogate applications, or donor prescreens, and it does not store new submissions through a website-native form. The only current online application is the Jotform egg donor application described below. If you email us, we receive the information you choose to include in that email. Please do not send health, medical, or genetic information through ordinary email.

Prospective egg donors may enter and submit information through the Jotform application embedded at /apply/egg-donor. The form may request identifying, contact, eligibility, or other information. Jotform is a third-party service; its notices, policies, and the form's settings govern its processing of information you submit there.

Website hosting, security, or other technical services may process technical information as part of operating the site. Which details are logged and how long they are retained depend on the relevant systems and their configuration; this policy does not state a specific log inventory or retention period.

We use information for purposes including:

• Responding to general questions and providing requested egg donor matching guidance.
• Reviewing egg donor information submitted through the embedded Jotform application and communicating about next steps.

Embedding Jotform does not by itself establish HIPAA compliance. Where HIPAA applies, collection and handling of protected health information require an eligible Jotform account with HIPAA features enabled, an executed Business Associate Agreement (BAA) where required, and appropriate access, notification, and integration settings. These account arrangements cannot be verified from this public website and must be confirmed by the form owner before collecting regulated information.

The Jotform egg donor application may ask for sensitive information relevant to the application. Submit information only through that application when requested. Do not send health, medical, or genetic information through ordinary email or a general inquiry.

If you have a confidential matching question that requires sensitive details, contact us first and request an approved private communication method. Review Jotform's notices and the form's disclosures before submitting information there.

The Perfect Gene is not a substitute for a healthcare provider, and submitting information through the website or Jotform does not create a patient-provider relationship. Any medical provider or fertility clinic you work with may have its own privacy notice governing information it receives directly.

The website does not currently offer native submission forms. This describes the current site and does not mean information submitted through earlier forms was deleted. If historical records are held, they are handled according to applicable operational and legal needs; this policy does not identify which historical records exist or state a retention period for them. Current egg donor application responses are submitted through Jotform; handling and retention there depend on Jotform's policies, the form configuration, and applicable arrangements. We do not state a fixed retention period for those submissions here. Technical logs may also be subject to the configuration and retention practices of the systems that generate them.

No internet transmission or electronic storage system is completely secure. Review the notices provided by Jotform and other services involved before providing information.

Depending on where you live, you may have rights to request access to, correction of, or deletion of personal information, subject to legal exceptions. You may also withdraw consent for optional communications or ask us about how your information is used.

To make a privacy request, email agency@theperfectgene.com. Do not include health, medical, genetic, or other sensitive information in an ordinary email request. If your request concerns an egg donor application submitted through Jotform, indicate that you need help with a Jotform submission and ask us how to identify it using an approved private method.

You can opt out of marketing emails by using the unsubscribe instructions in the message. Transactional or service-related communications may still be sent when necessary.

If you are dissatisfied with our response, you may have the right to contact the privacy regulator or consumer protection authority in your jurisdiction.

This website and our services are not directed to children under 13, and we do not knowingly collect personal information from children under 13. If you believe a child has provided personal information to us, please contact us so we can review and delete it when appropriate.

We may update this policy from time to time to reflect changes in our services, technology, or legal requirements. When we do, we will post the revised version here and update the “Last updated” date. Your continued use of the website after an update means the revised policy applies to your use going forward.
